Submit your Online Application
After completing the online application and paying the required service fee, you’ll be directed to our secure order page for further requirements and instructions.

Prepare your Documents
On your order status page, our system will generate your required forms and provide all the necessary information you need to mail to our processing center. Please note, it’s very important that you print the documents single sided; double sided copies will not be accepted.
Required Documents Checklist:
- DS-11 Passport Application: This form will be generated through our platform. Do not sign it until you’re at the Acceptance Agent’s office.
- Passport Photo: Click here for details on photo requirements.
- Proof of U.S. Citizenship: Click here for a list of acceptable documents.
- Proof of Identity: Click here to view acceptable forms of ID.
- Proof of Travel: Provide a travel itinerary or business letter showing you’re traveling within 2 weeks (or 4 weeks if a visa is also required). If you don’t yet have travel plans, we can help you obtain an acceptable substitute.
- Government Fee: A check or money order payable to “U.S. Department of State” is required. The amount typically ranges from $160 to $190 depending on your application type. The platform will guide you on the exact amount and how to complete the check.
- Letter of Authorization: You’ll receive two copies of this form from us. Sign and date both (preferably in blue ink). Place one copy inside the sealed envelope and one outside when sending us your documents.
Important: Make two photocopies of all supporting documents before submitting.
Passport Acceptance Agent
Once you’ve completed your passport application and gathered all required documents, you must bring them to a Passport Acceptance Agent (AA). The agent will review your application and witness your signature. Acceptance Agents are commonly found at U.S. Post Offices or Courthouses and are typically easy to locate in your area.
The Acceptance Agent will:
- Ask you to sign the DS-11 Form in their presence.
- Add their own signature to the DS-11 Form.
- Verify your identification.
- Collect all your supporting documents and place them, along with the signed DS-11 Form, into an envelope.
Important: Make sure the Acceptance Agent (AA) returns the sealed envelope to you—you must mail it directly to United Passport yourself. Send the sealed envelope as soon as possible, as the seal is only valid for four business days after it is applied.
Mail your Passport application package
- Before sending your documents, contact your assigned Account Manager for a quick review and to receive your pre-paid FedEx shipping label.
- Clearly write your full name, date of birth, and travel date on the sealed envelope.
- Place the sealed envelope inside a FedEx envelope.
- Include photocopies of all supporting documents and the DS-11 form outside of the sealed envelope but inside the FedEx envelope. This helps us verify everything for accuracy and avoid delays.
- Use the provided shipping label to send the FedEx envelope.
- Once your passport is ready, we will retrieve it from the government office and ship it to you via FedEx immediately.
