  • Both Parents or Legal Guardians must appear with child at a local acceptance agent
  • Notarized DS-3053 if one of the parents/legal guardians cannot be present

  • Your passport is valid but damaged

  • Your previous passport was issued when you were 16 or older
  • Your previous passport was issued in the past 15 years
  • You currently have your previous passport in your possession
  • You need a second passport
